PERFORMER FOR HIRE FORM 

The Performer For Hire page is an online portfolio available to the public to hire professional performers. A performer is a Middle Eastern dancer or musician—performing solo or with a group. 

Requirements: To be included on the Performer For Hire page you must be at a professional level and an active Guild member.  Renewal form at Guild website > Resources > Forms.
 

Note: If you are filling in information for multiple roles (soloist, musician, etc.), please fill out all the information below for each. Use words sparingly.
